General Information

Please note that sometimes one must play loose with the way this form is used. Consult Kitsufox if you feel you have an appropriate situation (Example: Pregnancy) for an alternate meathod of using the mild, moderate, and severe case sections.

Main Complaint Information

General Information

Human Name:
Provide the name humans use for the ailment. In some cases multiple names will need to be listed simply because cats lack the diagnosis technology to differential certain ailments from one another.

Description:
Provide an overview description of the ailment in single paragraph. Follow that with a short paragraph discussing symptoms.

Notes:
Make important statements concerning things players considering the illness may wish to know. Bold the key words and phrases.
Examples:
"This ailment is contagious via contact with the sick cat."
"This ailment has a high probability of fatality."


Mild Case Information

Duration:
Describe the typical duration of the illness using days/moons.
Example: "The patient should be restored to health within 2 days."


If followup treatment is required beyond the conclusion of the illness proper, separate this from the illness itself.

Example: "1 moon of intense treatment, followed by approximately 2 of follow-up treatments."

Treatment:
Describe the recommended treatment procedure for the complaint in a bulleted list.

Effects:
Describe the residual effects of the ailment, including the amount of time that can be expected to spend that way (in moons).
Examples:
"Weakness and Poor condition for approximately a quarter moon."
"Weakness of the break site for a half to full moon.
High probability of temporary lameness.
Low probability of permanent lameness."

Complications:
Describe the potential complications the illness might cause.
Examples:
"In the very young or very old additional infections are likely."
"High potential for damaging the same bone again if enough healing time is not taken.
Low potential for permanent damage to mobility."


Moderate Case Information

Describe the typical duration of the illness using days/moons in the fashion used for Mild Cases.

Treatment:
Describe the recommended treatment procedure for the complaint in a bulleted list.
There are times when differences in treatment are mild and one may use the "as mild" with the added items bulleted below that.

Effects:
Describe the residual effects of the ailment, including the amount of time that can be expected to spend that way (in moons).

Complications:
Describe the potential complications the illness might cause.
